The 2012/13 Chelsea Player of the Year awards ceremony takes place on Thursday 16 May, with tickets for the event on sale now.
Tickets are priced £200 including VAT, and can be purchased on this website or via the call centre on 0871 984 1905.
Included in the price of the ticket are the following:

The event will take place in the Great Hall at Stamford Bridge, with a reception at 6pm and dinner served at 7pm.
Supporters are now able to register their vote for the Player of the Year award, with voting for Goal of the Season to follow shortly.
Last season saw Juan Mata win the award following an impressive debut campaign, with Lucas Piazon, currently on loan at Malaga, taking the Young Player of the Year award.
With the Blues at the semi-final stage of the Europa League, and pushing to finish in the top four in the league, there is still time for players to stake a late claim, so make sure you back your favourite by registering your vote.
Peter Bonetti was the first winner of the award back in 1967, while from the current squad, John Terry, Frank Lampard, Petr Cech and Michael Essien are all past winners of the prize.
